Usually this is due to a partially blown speaker; a burn in the coil or
whiskers resulting in a near break in the circuit which is only
overcome by applying enough current (turning the volume up). Swap the
speakers; take the known working speaker and place it in the position
of the failing speaker, if the problem goes away, you've confirmed that
it is the speaker.
