Most common problem is that the pinch roller is slipping, and when the
supply reel diameter gets small, the pinch roller can't pull the tape.
Or -
check hold back tension
check than the brake isn't sticking
check that the shaft or motor holding the supply reel hasn't seized up
check the capstan

the capstan pulls the tape at a constant speed
the supply reel usually pulls against the tape with about 3 oz of tension
for 1/4 inch tape
the take up reel pulls in the direction of the tape also with about 3oz of
tension
